29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) On or about January 10, 2018, and at times prior, employees were exposed to eye injury and struck-by hazards from kickback, misfire, and flying debris while using pneumatic nail guns, and working adjacent to pneumatic nail guns, with out wearing safety glasses.     Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  On or about January 10, 2018, and at times prior, employees were exposed to falls from 21 feet to greater than 31 feet while reroofing apartment buildings without attaching their harnesses to ropes and anchors. 29 CFR 1926.502(d)(6)(i): Snap hooks were connected  directly to webbing, rope or wire rope:  (a)  On or about January 8, 2018, employees were exposed to falls when using a vertical  lifeline that wrapped around metal lattice on apartment buildings and the snap hook was attached to the rope.
