29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On January 10, 2018, located on the north side of a commercial exterior structure, employees were exposed to a fall hazard while standing inside a JLG Articulating Boom Lift basket Model# 450AJ installing 2 X 4 lumber onto a steep commercial pitch roof without a lanyard being attached to the boom or basket. Potential fall height was measured to be 23'-9".  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a) On January 9, 2018, located on the north side of a commercial exterior structure, an employee was observed working on a steep pitch roof ridge while wearing a harness that was not attached to an already installed temper anchor with rope and rope grab. Potential Fall Heights were measured to be 23'-9" with a roof pitch of 8/12.  b) On January 10, 2018, located on the north side of a commercial exterior structure, employees were observed installing temper anchors to a steep pitch roof ridge while wearing a harness that was not attached to an already installed temper anchor with rope and rope grab. Potential Fall Heights were measured to be 23'-9" with a roof pitch of 8/12.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.1053(b)(12): Ladders shall have nonconductive siderails if they are used where the employee or the ladder could contact exposed energized electrical equipment, except as provided in 1926.951(c)(1) of this part.  a) On January 9, 2018, located on the north side of a commercial exterior structure, employees were utilizing a Werner 32' aluminum extension ladder with conductive side rails to gain access to a steep commercial pitch roof located within 480 volts of an energized electrical power line.  b) On January 10, 2018, located on the north side of a commercial exterior structure, employees were utilizing a Werner 32' aluminum extension ladder with conductive side rails to gain access to a steep commercial pitch roof located within 480 volts of an energized electrical power line.
