29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about January 10, 2018, employees were exposed to a trench cave in. The employees were performing sewer line repair on a residential home located at 8159 Albion Road, North Royalton, Ohio 44133. An employee was working in a trench that was approximately 8 feet deep without any form of cave in protection.
