29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Top of framed house exterior walls - On or about January 10, 2018 employees framing the multi story house on a raised foundation were wearing a full body harness for fall protection but was not attached to a lanyard to keep them from falling off the structure that is 36 feet above the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Under center of house - On or about January 10, 2018 employees gained access to the raised house by using a wooden job made latter that was 4 feet short of the first floor landing. Employees had to climb up 2 feet onto wooden plank then jump up 2 feet to get on the first floor landing.

(a) East side of house next to street - On or about January 10, 2018 the employer had an employee operating an aerial lift that was within 10 feet of an energized power lines to lift and lower employees from the framed house.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  (a) At base of wooden ladder - On or about January 10, 2018 employees using wood ladder to gain access and exit the new single family dwelling being built had to step over scrap lumber lying on the ground that has nails protruding up through it.

(a) Under raised house - On or about January 10, 2018 employees framing a new multi story house on a raised foundation 18 feet high were using a job made wooden ladder with the spacing between the step risers at 16 inches.
