29 CFR 1910.28(b)(13)(i):  When work was performed less than 6 feet (1.6 m) from the roof edge of a low-sloped roof, the employer did not ensure each employee was protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.  Katz Library, Low-Sloped Roof - During snow removal operations, employees less than 6 feet from the edge of the roof were not protected from a fall hazard of approximately 13 feet by means of a guardrail system, safety net system, travel restraint system, or personal fall arrest system.

29 CFR 1910.28(b)(13)(ii):  When work was performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge of a low-sloped roof, the employer did not ensure each employee was protected from falling by using a guardrail system, safety net system, travel restraint system, personal fall arrest system; or a designated area when performing work that is both infrequent and temporary:  Katz Library, Low-Sloped Roof - During snow removal operations, employees at least 6 feet but less than 15 feet from the edge of the roof were not protected from a fall hazard of approximately 13 feet by using a guardrail system, safety net system, travel restraint system, personal fall arrest system, or by establishing a designated area.

29 CFR 1910.30(a)(1): Before any employee was exposed to a fall hazard, the employer did not provide training for each employee who was required to be trained as specified elsewhere in this subpart:  Katz Library, Low-Sloped Roof - Employees performing snow removal operations and exposed to a fall hazard of approximately 13 feet were not trained in the nature of the fall hazard and procedures to be followed to minimize the hazard.
