29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   A.)  Esmark Excalibur LLC dba Excalibur Machine - On or about, 1/12/18, and at times prior thereto, the company did not perform annual inspections on their lockout/tagout procedures with all authorized employees.  The employees performed servicing and maintenance on CNC's and presses.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   A.)  Esmark Excalibur LLC dba Excalibur Machine; Makino Area - On or about, 1/12/18, and at times prior thereto, a 208 volt panel box was used for the storage of a rolling cart.  The cart had supplies on it.  The panel box was used to power machinery in the area.
