29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized:  (a) 1438 W. Shunk St., Philadelphia, PA 19145 - The employer did not attach tags to the circuit breakers in the panel in the basement that were used to de-energize the outlet, lighting, and junction box circuits in the rear wall of the kitchen where workers installed drywall, thus exposing employees to contact with electrical hazards of greater than 50 volts.  This condition was observed on or about January 15, 2018.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) 1438 W. Shunk St., Philadelphia, PA 19145 - A written hazard communication program was not developed or implemented in the workplace where employees used hazardous chemicals, including, but not limited to:   -   Silica, contained in Quikrete Mortar Mix; and -   Portland cement, contained in Quikrete Mortar Mix.  Employees were thus exposed to the health hazards of the hazardous chemicals. This condition was observed on or about January 16, 2018.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) 1438 W. Shunk St., Philadelphia, PA 19145 - Hazardous chemicals were stored and used by employees in the worksite, but the employer did not provide training, thus exposing employees to hazards of the chemicals, including, but not limited to:   -   Silica, contained in Quikrete Mortar Mix; and -   Portland cement, contained in Quikrete Mortar Mix.  This condition was observed on or about January 16, 2018.
