29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    a)    Pentagon: On or about 01/16/2018, the Outback 145 Lincoln Electric 4750 Watts, three-wire single phase welder unit used to supply power to a Milwaukee portable band saw was not provided with a ground fault circuit interrupter or an assured equipment grounding conductor program, as a result employees were exposed to a possible electric shock hazard.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
