29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:    On or about 1/18/18, the employer failed to ensure that the walking working surfaces were free from slip/trip hazards. Areas include, but are not limited to the following:    a) Near the Daf unit, there were discharge pipes in the walkway, and    b) The floors were covered in water and raw material, and    c) On the mezzanine, surrounding the Vertical Washer, there was a floor opening that was about 28.5" in width (nearest to the Vertical Washer), and 6" in width (furthest from the Vertical Washer), with a length of about 13.75" from the Vertical Washer to the walking/working surface. The depth from the walking/working surface to the pipes below was about 21".
