29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects.  On or about 23 January 2018, and at times prior thereto, two employees were changing light bulbs from light poles without the use of hard hats or head protection, exposing the two workers to the hazard of being struck by falling tools, materials, equipment, or debris.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  On or about 23 January 2018, and at times prior thereto, a worker was using an aerial lift to perform elevated work on light poles without the use of fall protection, exposing the worker to a fall hazard of approximately 25 feet to the pavement below.
