29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.  On or about 23 January 2018, and at times prior thereto, a mud pan was elevated by a JLG Rough Terrain Forklift, Model 6042, to the 2nd level of a tubular-welded frame scaffold where a mason stepped from the scaffold into the pan to mix and scoop out mud for stucco work. The pan was non-descript and unattached to the fork mast or tines. The mason was exposed to a fall hazard of approximately 11 feet 5 inches to the ground.
