On or about 1/25/2018  a) the employer did not ensure employees who operated forklifts daily were trained and certified.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l).  The certification shall include the name of the operator, the date of the training, the date of the evaluation,  and the identity of the person(s) performing the training or evaluation.  Site: 517 7th Avenue, Brooklyn, NY.  On or about 1/25/2018  a) An employee operated a forklift daily on the worksite and the employee was not trained and certified.
