29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    a.  On or about January 25, 2018, at the site located at 368 Brets Lane, Lewis Center, Ohio, employees were installing siding on a two story residential home from a pump jack scaffold without the use of fall protection, thereby exposing the employee to a fall hazard of 15 feet.

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;     a.  On or about January 25, 2018, at the site located at 368 Brets Lane, Lewis Center, Ohio, an employee was installing siding on a two story residential home from a chicken ladder without the use of fall protection or a grab line, thereby exposing the employee to a fall hazard of 12 feet.

On or about January 245 2018, at the site located at 368 Brets Lane, Lewis Center, Ohio, the employer failed to provide retraining to an employee that demonstrated a lack of proficiency in safe scaffold use.
