29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a- 210 - 5th Avenue, Belmar NJ: Workers were exposed to fall hazards, in excess of 17 feet to the ground surface below, while installing siding and performing other related activities from a pump jack scaffold that was missing end stops and a mid rail/screen.  Condition observed on or about 01/25/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a- 210 - 5th Avenue, Belmar NJ: Company employees were exposed to fall hazards, in excess of 10 feet to the ground surface below, when accessing a ladder while walking on a low pitched roof without the use of fall protection.  Condition noted on or about 01/25/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a- 210 - 5th Avenue, Belmar NJ: Company employees were exposed to fall hazards, in excess of 10 feet to the ground surface below, when accessing an aluminum extension ladder whose side rails did not extend at least three feet above the upper landing surface being accessed.  Condition noted on or about 01/25/18.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
