29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) 1738 W. Tilghman St. Allentown, PA: On or about November 7, 2017, two employees were installing sheets of plywood while working from the basket of an articulating boom aerial lift, positioned up to approximately 15 foot above grade, without wearing a body belt and lanyard attachment attached to the boom or basket.
