On or about January 26, 2018, at the above addressed jobsite, employees conducting masonry operations from a tubular welded scaffold scaffold was not trained to recognize the hazards associated with scaffolding.  The employees were thereby exposed to the hazards associated with falls.

On or about January 26, 2018, at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, twelve (12) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.
