29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103) a) 401 7th North St, Syracuse, NY , On or about 1/25/18: The employer failed to implement a Written Respiratory Protection Program for an employee, who was using an N95 respirator, while chipping concrete that encompassed the following: 1) A medical evaluation to ensure ability to wear a respirator. 2) A fit test to ensure a proper fit. 3) Training of employees in the respiratory hazards to which they were exposed during routine and emergency situations.
