29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.     a. On or about January 26, 2018, employees engaged in construction activities at the above referenced inspection site were working in areas requiring personal protective equipment to safeguard against falls from heights, loss of eye sight, overhead hazards, and hazards associated with material handling operations. The employer failed to ensure all employees proficient in recognizing these hazards as required. Employees were thereby exposed to falls from heights, stuck-by, caught-in, and crushing hazards.     b. On or about January 26, 2018, at the above referenced inspection site, employees engaged in material handling operations were not trained to recognize the hazards associated with free rigging.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about January 26, 2018, employees engaged in construction activities at the above referenced inspection site were working in areas requiring personal protective equipment to safeguard against overhead hazards, and hazards associated with material handling operations. The employer failed to ensure all employees utilized the necessary personal protective equipment as required. Employees were thereby exposed to stuck-by hazards likely to result in head injuries.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

As fire hazards occur, there shall be no delay in providing the necessary equipment.    a. On or about January 26, 2018, employees engaged in construction activities at the above referenced inspection site were working in areas with the propensity for fire. The employer failed to establish a fire protection program as required by this standard.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a. On or about January 26, 2018, at the above referenced inspection site, employees working on the second floor deck in the material handling area, approximately fourteen feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
