29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  Alpha Coatings, Inc. - Fostoria, Ohio: On or about January 17, 2018, the employer did not report a hospitalization of an employee following a work-related injury.  On January 4, 2018, an employee was hospitalized following a same-level slip and fall incident; the employer did not report the hospitalization within 24 hours.
