On or about January 30, 2018, the employer did not ensure energy control procedures were developed, documented, and utilized before performing maintenance tasks on the SILLA die cutter in the offline packing area (Serial #2015-6-170). Employees are required to set the die and are exposed to crush hazards from the cylinders on each side of the open door on the front of the die cutter.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a.) On or about January 30, 2018, the employer did not ensure employees were protected while performing maintenance tasks, such as but not limited to changing dies on the SILLA die cutter in the offline packing area (Serial #2015-6-170). Employees are required to set the die and are exposed to crush hazards from the cylinders on each side of the open door on the front of the die cutter.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: On or about January 30, 2018, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as, changing dies on the SILLA die cutter in the offline packing area (Serial #2015-6-170) and were exposed to crush hazards from accidental startup of the die cutter, in that the equipment was not properly deenergized.
