29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) The west side of a commercial building:  On or about January 31, 2018, employees without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below, while installing steel bars on the roof of a commercial building.    b)The east side of a commercial building:  On or about January 31, 2018, employees without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below, while installing steel bars on the roof of a commercial building.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about January 31, 2018, employees were exposed to fall hazards greater than 6 feet to the paved ground below, while installing steel bars on the roof of a commercial building without being trained to recognize fall hazards.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.      29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d)    On the west side of the commercial building:  On or about January 31, 2018, an employee was exposed to an ejection and/or crushing hazard, while operating a JLG forklift, Model 644E-42, prior to receiving training, evaluation and being certified competent to operate a powered industrial truck.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) The west side of a commercial building: On or about January 31, 2018, employees were exposed to fall hazards, when using a ladder that was less than 3 feet above the upper landing surface, while gaining access to the roof.    b) The east side of a commercial building: On or about January 31, 2018, employees were exposed to fall hazards, when using a ladder that was less than 3 feet above the upper landing surface, while gaining access to the roof.
