29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction on Lot #15, East side: On or about 01/31/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that employees were not provided with protective eyewear while operating these tools engaged in siding installation.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) New residential construction located on Lot #15, East side at cut bench: On or about 01/31/2018, and at times prior the employer exposed employees to amputation hazards while operating circular saws, in that the circular saws are used with the lower guard fixed in the upward position with a wood shim which exposes all users to amputation hazards.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a) New residential construction located on Lot #15, East side: On or about 01/31/2018, and at times prior, the employer exposed employees to collapse/fall hazards on a pump jack scaffold, in that the scaffold was made longer on both side using Micro-Lam structural boards secured by a tie-down strap which extended the scaffold 3-feet on each side. The overlapped did not occur over a support which exposed employees to collapse/fall hazards of up to 16-feet, 2-inches to the lower level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) New residential construction located on Lot #15, East side: On or about 01/31/2018, and at times prior, the employer exposed employees to fall hazards of up to 16-feet, 2-inches to the lower level, in that a fall protection system was not used while employees were installing siding from a pump jack scaffold system.
