29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Northwest corner of roof, on or about 2-1-18:  Three employees were working on a flat roof with unprotected sides and edges, not protected from falls, exposing the employees to a fall hazard of 22 feet.  ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a) Northwest corner of roof, on or about 2-1-18:  Three employees were accessing a roof by stepping/jumping from a parapet to the roof, with a change in elevation of 24 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) West side of roof, north corner, on or about 2-1-18:  Three employees were using a fiberglass extension ladder for access to a roof, with side rails that were 27 inches above the landing surface, exposing the employees to a fall hazard of  22 feet.
