29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:  On or about February 1, 2018, at the above addressed site, an employee performing electrical work installing fluorescent light bulbs in an existing sign was exposed to a fall hazard of approximately 8 feet 6 inches to 11 feet when working from the basket of an aerial lift while not having a lanyard attached to the D-ring on his harness and the anchor point provided on the platform of the lift.
