29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Top plates, 2nd level walls: On or about February 1, 2018, the employer did not ensure fall protection was used while walking and marking the top plates which exposed employees to falls up to 22-feet.  b) Decked 2nd level floors: On or about February 1, 2018, the employer did not ensure fall protection was used while walking and working on the floors with unprotected edges which exposed employee to falls up to 13-feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Portable Extension Ladder, metal: On or about February 1, 2018, the employer did not ensure the ladder was setup with at least 3-feet of side rails above the upper landing surface to assist employees gaining access to the 2nd level of the residential structure which exposed employees to falls up to 13-feet.
