29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) On February 1, 2018 and times prior, the employer had not developed a written energy control program consisting of machine specific lockout/tagout procedures, employee training and periodic inspections of the program and procedures. As a result, employees were exposed to amputation, serious struck-by and or crushing injuries where the unexpected energizing, startup or release of stored energy could occur.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:      (a)  On February 1, 2018 and times prior, the employer did ensure that complete periodic (annual) inspections were conducted for the 3-Ton Harrington Overhead Hoist/Cranes, Serial numbers 00120963 and 00132293 located in the Sign Assembly and Loading Areas. The equipment had been installed and certified in March 2013, however, the company was unable to produce any inspection records beyond that year.
