29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    SPN Rig #20 V-Door area. On or about January 30, 2018, standard guardrail systems or other fall protection systems were not in place near the V-Door area. Employees were exposed to a fall from elevation of more than 4 feet while performing cementing work.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    SPN Rig # 20. On or about January 30, 2018, and at times prior thereto,  the employer did not ensure that flame resistant clothing (FRC) was on all outermost layers of employee's clothing when working in areas where there was a potential for hydrocarbons to be present, exposing the employees to fire and explosion hazards.

Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location.    SPN Rig #20. On or about January 30, 2018 and at times prior thereto, Dayton Portable Electric Heaters used on the drilling rig floor were not approved for a Class I Division II location.
