29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  a). At the worksite - On or about and times prior January 31, 2018, the employer failed to instruct employees on how to recognize and control hazards associated with roofing activities, extension ladders, and pneumatic tools, thus exposing employees to fall and struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a). At the worksite - On or about and times prior to January 31, 2018, an employee operated a Hitachi pneumatic nail gun without the use of eye protection, thus exposing the employee to a struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a). At the worksite - On or about and times prior to January 31, 2018, employees were working on the roof conducting re-roofing activities without the use of a fall protection system in place, thus exposing employees to a fall hazard up to approximately 20.6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a). At the worksite - On or about and times prior to January 31, 2018, the side rails of two Werner portable extension ladders were not extended at least three (3) feet above the roof's landing surface, thus exposing employees to a fall hazard up to approximately 13 feet.
