Employees were exposed to fall hazards of up to 7 feet while operating the color printing unit standing on a platform with missing guardrails. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a) On or about February 1, 2018, and at times prior, the employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a series of MGD GOSS printing presses where the unexpected energizing, startup or release of stored energy could occur and cause injury, the printing presses were not isolated from their energy source and were not rendered inoperative. Employees were exposed to nip-point and caught-in hazards while cleaning, adjusting, and/or replacing the printing rollers without isolating the energy source on the machines.
