29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about January 30, 2018 at Lot next to 25 Crescent Park, Pike Road, Al: The employer failed to ensure an employee decking the second floor porch used fall protection which exposed him to falls of 14 feet 2 inches to the ground.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  a) On or about January 30, 2018 at Lot next to 25 Crescent Park, Pike Road, Al: The employer failed to ensure an employee using personal fall arrest was properly anchored in that his rope was wrapped around a beam and the snap hook was connected back to the rope while installing the joist for the pouch.
