29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about January 31, 2018 at 76 Main Street, Pike Road, Al: The employer failed to ensure an employee installing metal roofing on a new residential house wore proper fall protection in that system was missing a deceleration device connected to his rope grab and then connected to his harness with a snap hook.  Instead he used a piece of rope and connected it to the rope grab by tying a knot and the other end was tied to the harness by a knot.  b) On or about January 31, 2018 at 76 Main Street, Pike Road, Al: The employer failed to ensure an employee installing metal roofing on a new residential house wore proper fall protection in that his deceleration device had deployed and was not replaced.
