The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  On or about February 8, 2018, the employer did not document the required certification for each operator which includes; the operators name, date of training, date of evaluation and/or the name of the person conducting the evaluation.
