The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards:  On or about 2/1/2018;  Location: Inside Prologis building,   1) Employee fireproofing ceiling pipes at a height of approximately 15 ft was using the fly section of an extension ladder to reach the pipes. The employee was not trained properly and was unable to recognize the hazards related to improper use of ladders.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  On or about 2/1/2018;  Location: St.Barnabas Hospital,   1) The injured employee was admitted to the St. Barnabas Hospital, and was diagnosed with left wrist (distal radius) fracture. A sugar-tong splint was applied and it was explained to the patient that he would need surgical repair. Employer failed to report the in-patient hospitalization to OSHA.
