29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  F/V Aleutian Sable, Resoff Dock, walk-in freezer; on or about 08 February 2018 and time prior thereto; the employer did not ensure that safety features were in proper working order at all times. The emergency escape plunger on the walk-in freezer was missing the internal plunger element. This condition exposed employees to a caught in hazard.

F/V Aleutian Sable, Resoff Dock, Deck Port side, on or about 08 February 2018 and time prior thereto; A Skilsaw brand 7 1/4-inch Worm Drive Saw, Model HD77M, Serial Number H18327, had damage to the outer protective jacket. This condition exposed employees to an electrical shock hazard.

29 CFR 1904.32(a)(4): The employer did not post an OSHA 300A Form or equivalent by February 1 thru April 30.  F/V Aleutian Sable; on or about 08 February 2018 and time prior thereto; the OSHA Form 300A for the calendar year 2017 was not post. This condition leads to inaccurate reporting of injuries and illnesses.  Arctic Sablefish LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.32(a)(4) which was contained in OSHA inspection number 1041845, citation number 2, item number 2 and was affirmed as a final order on 04 May 2015, with respect to a workplace located at Coastal Dock in Dutch Harbor, AK.

This condition exposed employees to unsafe work practices.
