An employee fell approximately 16 feet through one of the holes that was covered by an unsecured cover, on or about, February 9, 2018.  There is no abatement certification or documentation required for this item.

An employee fell approximately 16 feet through one of the holes that was covered by an unmarked cover, on or about, February 9, 2018.
