OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the exit door provide for emergency egress in the Breeding Room:   The employer is failing to ensure that the door provided for emergency exit in the breeding room could be readily opened from the egress side exposing employees to inhalation hazards from delayed exit during a fire or nocuous gas release.  The door was secured (locked) in such a manner as to require special knowledge or effort for operation from the egress side; and the fastening device a looped wire rope around the door handle, did not provide a releasing method that allowed opening of the door leaf with not more than one operation in accordance with NFPA 101.     Among other methods, feasible and acceptable methods of abatement:  NFPA 101 prescribes three performance-based provisions to determine whether doors used as a means of emergency egress can be latched or locked. The three provisions are:   (1) Doors used as a means of egress shall be arranged to be opened readily from the egress side whenever the building is occupied;   (2) Locks, if provided, shall not require the use of a key, a tool, or special knowledge or effort for operation from the egress side;  (3) A latch or other fastening device shall be provided with a releasing device that opens the door leaf with not more than one operation.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer failed to ensure that employee(s) were not exposed to an atmosphere that was immediately dangerous to life or health(IDLH).      The employer is failing to protect employees from IDLH atmospheres. This was most recently documented on 31 January, 2018 at 56265 702nd Rd. Mahaska Kansas. Employee(s) were exposed to hydrogen sulfide levels that were above IDLH within the restroom adjacent to the breeding room.The hydrogen sulfide was introduced into the restroom via a floor drain located within the restroom that was connected to the manure pit located under the hog confinement(s).        Among other feasible and acceptable methods of abetment:    (1) Ensure that the sewer line connected to the floor drain within the breeding room/restroom is equipped with a functioning p-trap. The employer shall ensure the floor drain p-trap has a liquid seal of not less than 2 inches (51 mm) and not more than 4 inches (102 mm), or deeper for special designs relating to accessible fixtures.  International Plumbing Code 2009 - Chapter 10 Traps, Interceptors and Separators Section 1002.4.
