29 CFR 1917.91(a)(1): The employer did not ensure the use of eye and/or face protection equipment when employees were exposed to eye and/or face hazards:    LTL Warehouse:  On or about February 9, 2018 and at times prior thereto, employees were not using eye and face protection when operating high pressure washing equipment.  This condition exposed the employees to eye injuries.

29 CFR 1917.154: Compressed air was used to clean employees:    Shop, Bay 3/4:  On or about February 9, 2018 and at times prior thereto, compressed air rated at 140 p.s.i.g. was used by an employee to blow himself down prior to break. This condition exposed the employee to air embolism hazards from direct contact with the employees skin surface.
