29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Jobsites: All employees had not received information and training on the hazards of working in excavations and controls to  eliminate such hazards such as but not limited to safe means of egress for excavation 4 feet or deeper and  appropriate cave in protection when they may have to work in excavations 5 feet or deeper.

29 CFR 1926.651(c)(2): Means of egress from trench excavations.  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  Jobsite: There was no ladder in an excavation over 4 feet deep to provide for safe egress where employees were digging out rocks and dirt.
