(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:     Tanker Truck Cleaning Area:  On or about February 15, 2018, and times prior thereto, employees were exposed to fall hazard greater than 4 feet to the ground level below when power washing from the top of tanker trucks without a fall protection system in place.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Tanker Truck Cleaning Area:  On or about February 15, 2018, and times prior thereto, employees were exposed to a splash hazard while power washing tanker trucks without any personal protective equipment for the eyes and face.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   In the Truck Wash Area: On or about February 15, 2018, employees were not wearing personal protective equipment such as but not limited to: face shield, personal fall protection system were exposed to waste water/sewer sludge/ grease when working from the top hatch opening of the tanker truck while power washing.
