29 CFR 1926.501(b)(13): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems.  On or about February 14, 2018, an employee was working on a residential home without using any form of fall protection. This condition exposed the employee to fall hazards of more than six feet to the ground below.

29 CFR 1926.503(c)(3): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section.    On or about February 14, 2018, employees were working on a residential home without having adequate understanding and skill in using fall protection equipment. This condition exposed employees to fall hazards greater than six feet.
