The employer did not ensure that the step-across distance from the centerline of the rungs or steps for the through ladder, was not less than 7 inches (18 cm) and not more than 12 inches (30 cm) to the nearest edge of the structure, building, or equipment accessed from the ladders:    Employees are exposed to fall hazards.  Employees performing maintenance and inspections of air handling units 4,5 and 24 on elevated platforms were exposed to fall hazards of approximately 12 feet from the ladder to the floor in that the step-across distance from the ladder to the platform exceeds 12 inches.

A minimum perpendicular distance of 30 inches (76 cm) from the centerline of the steps or rungs to the nearest object on the climbing side. When unavoidable obstructions are encountered, the minimum clearance at the obstruction may be reduced to 24 inches (61 cm), provided deflector plates are installed (see Figure D-5 of this section).     Employees are exposed to struck by hazards.  Employees performing maintenance and inspections of air handling units 7 and 8 on elevated platforms were exposed to struck by hazard while climbing ladders in that there is an obstruction located closer than 30 inches to centerline of ladder.

Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.     Employees are exposed to fall hazards.  Heating Ventilation and Air Conditioning technicians working throughout building 2306, located at 2306 East Banister Road in Kansas City, Missouri are exposed to fall hazards of approximately 12 feet while performing maintenance and inspections of air handling units on elevated platforms in that the ladder opening was not protected. Exposure to this hazard was discovered in the following locations:       a)Air handler #9 approximately 22 inches of unprotected opening at the ladder opening of the platform.         b)Air handler #7/8 approximately 15 inches of unprotected opening at the ladder opening of the platform.         c)Air handler #4/5 approximately 12 inches of unprotected opening at the ladder opening of the platform.         d)Air handler #6 approximately 24 inches of unprotected opening at the ladder opening of the platform.         e)Air handler #1 approximately 30 inches of unprotected opening at the ladder opening of the platform.         f)Air handler #2/3 approximately 24 inches of unprotected opening at the ladder opening of the platform.         g)Air handler #34 approximately 20 inches of unprotected opening at the ladder opening of the platform.         h)Air handler #14/15 approximately 10 inches of unprotected opening at the ladder opening of the platform.         i)Air handler #25 motor side approximately 30 inches of unprotected opening at the ladder opening of the platform.         j)Air handler #28/29 approximately 20 inches of unprotected opening at the ladder opening of the platform.         k)Air handler #32/33 approximately 30 inches of unprotected opening at the ladder opening of the platform.         l)Air handler #35 approximately 24 inches of unprotected opening at the ladder opening of the platform.         m)Air handler #38 approximately 24 inches of unprotected opening at the ladder opening of the platform.         n)Air handler #18 approximately 30 inches of unprotected opening at the ladder opening of the platform.         o)Air handler #24 approximately 24 inches of unprotected opening at the ladder opening of the platform.         p)Air handler #10/11 approximately 12 inches of unprotected opening at the ladder opening of the platform.

The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11 of this section).    Employees are exposed to fall hazards.  Heating Ventilation and Air Conditioning technicians working throughout building 2306, located at 2306 East Banister Road in Kansas City, Missouri are exposed to fall hazards of approximately 12 feet while performing maintenance and inspections of air handling units on elevated platforms in that the top rails of the guardrail system did not extend at least 42 inches above the platform.
