29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extended at least 3 feet above the upper landing surface:  West side of convenience store:  On February 16, 2018, employees were exposed to fall hazards, while using a Louisville A-Frame Ladder that was less than 3 feet above the roof top of the building.

29 CFR 1910.23(c)(13):  The employer did not ensure that ladders were not placed on boxes, barrels, or other unstable bases to obtain additional height:   West side of convenience store:  On or about February 16, 2018,  an employee was exposed to a fall hazard while using a Louisville A-Frame Ladder which was setup on the back of a truck tailgate to give additional height for access and egress to the roof top of the building.
