29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.    (a) Dianal America Inc facility: On February 12 2018 and at times prior to, the employer did not document that the facility siting of the control room complied with Recognized and Generally accepted Good Engineering Practices (RAGAGEP) when the employer failed to do any of the following;   (I) Conduct building siting evaluations for existing buildings intended for occupancy  (II) Evaluate how the control room could be impacted in the event of a blast  (III) Evaluate how the control room could be impacted by flammable vapor release.    This condition exposed employees to fire and explosion hazards.   (Example RAGAGEP for facility siting include, but not limited to API 752, paragraph 5.4,6.6 and 7.4)

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)Dianal America Inc's facility: On February 12,2018 the employer did not develop, document and utilize procedures for the control of hazardous energy before conducting an inspection on a 16"  KITZ motor operated gate valve. This condition exposed employees to amputation hazards.
