29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of this part, the employer must ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level is protected from falling by: (i) Guardrail systems; (ii) Safety net systems; or (iii) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.    At the Hazel Lane Lease well site, the employer did not ensure employees who accessed and worked from the work-over rig rod landing and rod basket areas were provided fall protection exposing them to unprotected fall hazards of approximately 75 feet.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident.    The violation occurred on February 14, 2018, the employer failed to report to OSHA a work related fatality prior to the elapse of eight (8) hours reporting period of the incident.
