29 CFR 1926.305(c): When it was necessary to provide a firm foundation, the base of the jack was not blocked or cribbed, and a wood block was not placed between the cap and the load where there was a possibility of slippage of the metal cap of the jack: a) 2842 W. Girard Ave., Philadelphia, PA 19130, outside rear of building - The employer did not ensure that the base of the hydraulic floor jack was blocked or cribbed when in use to provide a firm foundation nor was there a block of wood placed between the cap and the load to prevent slippage thus exposing employees to struck-by and fall hazards.
