29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) Oceanport LLC -  On or about February 22, 2018,  the employer failed to provide appendix D for employees who voluntarily wore disposable N95 respirators while shoveling perlite, and salt in the hull of the ships.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) Oceanport, LLC - On or about February 22, 2018, the employer failed to ensure that all employees were trained and evaluated in the operation of the Caterpillar 953B Track Loader.
