29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    On or about February 22, 2018 employee(s) were permitted to operate the TCM forklift with fork extensions in-place without updating the lift per the standard requirements.

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about February 22, 2018 the employer did not ensure each employee authorized to operate the TCM forklift had completed training consisting of a combination of formal instruction, practical training and an evaluation of the operators performance prior to being permitted to operate the forklift.
