29 CFR 1910.333(b)(2): "Lockout and Tagging." While any employee is exposed to contact with parts of fixed electric equipment or circuits which have been deenergized, the circuits energizing the parts shall be locked out or tagged or both in accordance with the requirements of this paragraph. The requirements shall be followed in the order in which they are presented (i.e., paragraph (b)(2)(i) first, then paragraph (b)(2)(ii), etc.). a) Steam Generator Room: On or about February 6, 2018, and at times prior, the employer exposed employees to electrical shock hazards in that no specific procedures to control hazardous electrical energy were developed for conducting electrical maintenance on the Water Steam Generator (Model NB-55562 Serial Number 94249).

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) The employer did not notify OSHA of an incident that occurred on February 6, 2018 requiring the hospitalization of an employee until three days later on February 9, 2108.
