29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Addison Subdivision Lot 20: Top of front Porch: On or about February 22, 2018, and at times prior, the employer exposed employees to struck-by hazards while using pneumatic roofing nail guns, in that protective eyewear was not provided to employees operating these tools at 120 PSI while engaged in siding installation activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Top of the roof,  Lot Number 20 Addison Subdivision: On or about February 22, 2018, the employer exposed employees to fall hazards of approximately 20 feet while installing siding material on the front fascia of a single family construction site without proper fall protection.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.  a) Top of the porch, Lot Number 20 of Addison Subdivision: On or about February 22, 2018, the employer exposed an employee to fall hazards of approximately 11 feet while installing siding material using a ladder which did not have a stable and level surface.
