29 CFR 1926.100(a): At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats. On or about February 26, 2018, the employer failed to ensure employees who were working at a residential roofing job site were wearing protective head gear (hard hats). Employee were exposed to hazards such as but not limited to flying nails from electric nail guns, flying pieces and debris from removing and replacing the roof, part of its foundation and gutter work.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about February 26, 2018, the employer failed to ensure that employees who were working on a residential rooftop (approximate height at the access point via a portable ladder was 10.6 feet) were protected from a fall from height hazard. Employees were not wearing a personal fall protection harness nor were any other fall protection measures in place to protect employees from a fall.

The ladder was actually observed flush with the roof access point.
